Beyond Patient Care to Population Health sets out the Trust's approach to improve the health, wellbeing and care of people in Tameside and Glossop.
The approach focuses on:
- Preventing ill health and tackling inequalities
- Ensuring that high quality services work together to provide support when people do become unwell
- Developing a sustainable workforce fit for the future
- Working in partnership across the system to wrap services around the needs of our population
