About our service
The cardiac rehabilitation service forms part of the wider cardiology provision at Tameside & Glossop Integrated Care NHS Foundation Trust.
We are a multidisciplinary team that delivers an individualised exercise, education and support programme built around your personal circumstances and needs.
We offer rehabilitation for people recovering from various cardiac events, conditions and procedures. We also take referrals for people living with heart failure who need support with healthy lifestyle changes and exercise.
Early stage
In-hospital/early post discharge support following your cardiac event or procedure. At this stage you may be identified as appropriate and referred to cardiac rehabilitation. Our cardiac rehabilitation nurse will see you on the ward or contact you to offer support around medications and any questions you might have since discharge.
Core
Structured group rehabilitation sessions, including exercise, education, and personalised goal settings with the option of a home exercise programme rather than face to face classes. Sessions are designed to gradually and safely improve your fitness. You’ll be supported by Physiotherapists and support staff who monitor progress and adapt exercise to your level. Group classes vary on level of ability and run for an 8-week duration with one class a week.
Late stage
At the point of discharge from our service, we strongly encourage people to continue to complete their exercises at home as well as offering patients a referral to the Live Active team in Tameside or the Live Life Better Derbyshire team for Glossop patients. Both teams specialise in supporting people to continue exercise in the long term, running their services through local leisure centres.
Education and Lifestyle Support
As part of your rehabilitation, we run education sessions on various topics centred around heart health and healthy lifestyle choices.
Cardiac rehabilitation is a vital part of your recovery. Research has shown that cardiac rehab can reduce your risk of having another heart event or being readmitted to hospital. It also has a positive impact on your wellbeing and quality of life. Our goal is to support you in making a safe and confident recovery.
Where are we
We deliver rehabilitation for people living with long covid. We offer support and advice for people with symptoms of long covid such as fatigue and breathlessness.
You will be referred to physiotherapy services after you have had an assessment with the long covid consultant or nurse practitioner. Following this you will have an appointment with a physiotherapist, and a plan will be agreed based on your needs to support you with your recovery. Advice may be focused on activity and exercise, energy management, breathing and relaxation exercises.
Your assessment will take place at Ashton Primary Care Centre, and our contact number is 0161 922 6617.
