On 5 October, our Hospital at Home team launched a number of service enhancements designed to improve accessibility, streamline referral pathways, and strengthen the quality and safety of patient care.
Hospital at Home is an acute clinical service that delivers hospital-level care, staff, equipment, and treatments directly to a patient's home instead of an inpatient hospital ward
Developments include the new Urgent Response Single Point of Access (SPoA), providing a single point of contact for GPs, the ambulance service, care homes, NHS community teams, and other primary care clinicians seeking urgent clinical triage and advice for adult patients in the community.
Supported by senior clinical advisors within the Digital Health team, the SPoA simplifies access to services and helps ensure patients receive the right care, in the right place, at the right time.
In addition, referrals from the acute hospital to the Virtual Ward are now managed through the Community Clinical Portal on TIS, improving referral quality, consistency, and patient safety through a streamlined digital process.
These changes demonstrate the Hospital at Home Directorate’s commitment to delivering high-quality, accessible, and coordinated care for our patients and partners.
