About our department
The Community Gateway is a team of non-clinical staff who are the first point of contact for members of the public, referrers and agencies for district nursing matters.
The main roles and responsibilities of the Community Gateway are to:
- be the first point of contact for service users making an initial contact to the Community Gateway regarding District Nursing health matters
- co-ordinate incoming referrals
- be able to direct service users to the appropriate team or department dependant on their needs
- be able to offer re-direction to other services and provide relevant information where appropriate.
The non-clinical staff within the Gateway includes:
- Senior Administration Supervisor
- Community Gateway Supervisors
- Community Gateway Clerical Officers
Referrals for District Nursing can be taken over the telephone by the patient/healthcare professional/consenting carer or guardian by calling 0161 922 4888.
Referrals for District Nursing sent from Tameside & Glossop Acute Hospital must be sent via the EIS portal which can be found on the TIS page.
Referrals for District Nursing being sent from Tameside & Glossop GP Practices must be sent on the appropriate referral form via GP to Community referrals through EMIS web.
Referrals for District Nursing sent from a Tameside & Glossop Community Service must be sent on the appropriate referral form via Community to Community referrals through EMIS web.
Referrals for District Nursing being sent from external organisations can be received via email on the appropriate referral form to dngatewayreferrals@tgh.nhs.uk.
